I thought you couldn't progress the game (meaning the Pilika event in Toto doesn't trigger) until you recruit her.

To trigger the scene with Pilika, you need to visit Leona and change your party at least once before going to Toto.

Not sure about that. I remembered somebody tried to do a minimalist run, I don't know exactly who he is and what happened. But during his minimalist run, his Sindar Ruins party consisted only of Riou, Nanami, Jowy, and Gengen.

As far as I know... Millie is missable only in the Japanese version.

Might have been a bug that made her missable in the Japanese version, I don't know. There could be lots of reasons, really.
